SHE HAS RETURNED HER PERMIT ON THIS / 1ST DAY OF JULY 1987 AND WILL NOT / ATTEND ANY OTHER CASES.]. It is signed at lower right by the directors of the Bureau of Maternal and Child Health and the local health department. The back is blank with no printing, marks, or inscriptions.
