Cover of folded sheet bears the logo of the Artists' Union, a hand holding three paintbrushes in between the letters A and U. <br>Text on the cover reads: The Artists Union, 60 West Fifteenth Street, painters, sculptors, graphic artists, membership card. <br>Interior, left-hand side has printed lines for name, address, craft, employed, unemployed. In blue ink, Gottlieb has filled in: Harry Gottlieb, 6 East 14th St., Painter, and has checked "unemployed." Right-hand side has a table for tracking dues paid.
